Search for clinic management software in India and almost every result is a cloud product: you log in through a browser, your records sit on the vendor's server, and you pay every month. That is a good arrangement for a clinic in a city with fibre internet and a doctor who wants to see appointments on the phone. It is a poor one for a clinic in a small town where the line drops twice a day and the reception computer is the only computer that matters.

What a clinic actually uses every day
Most clinic software is sold on a long list of modules. In practice, a clinic's day comes down to a handful of jobs, done fast, with a queue watching:
- Registering the patient. Name, age, gender, mobile number and address on one screen. A UHID is made automatically, and a returning patient is found by name, mobile number or UHID, with every earlier visit attached.
- Taking the consultation fee. Choose the doctor and the fee fills itself in. Cash or UPI is recorded against the visit, and the OPD slip prints with a barcode and space for vitals.
- Billing tests. Pathology and radiology from your own price list, several tests on one bill, with a discount when you give one.
- Patients who only come for a test. They are billed as outside patients, so your OPD register and doctor-wise counts stay clean.
- Closing the cash. Every fee, lab bill and expense goes into the cashbook as it happens, with cash and UPI kept apart. At night you check the total against the drawer instead of adding up slips.
If your clinic also admits patients, the same software handles admissions, charges during the stay, part payments and the discharge bill. That is the part nursing homes buy it for, and it is included in the same price.
Why offline matters more than it sounds
When a cloud system loses its connection, the counter stops. Patients wait while someone switches to paper, and later that paper has to be typed in again, which is where bills go missing. An offline system simply carries on. The internet is used for three things only, and all of them can wait until the line comes back: activating the licence once, the daily backup to your Google Drive, and checking for updates.
There is a second reason, and it is about records. With Adhrit HMS nobody else holds your clinic's patient data. If you stop paying, your records stay readable, printable and exportable on your own computer. With many cloud products, access to the records ends with the subscription.
Printing on your own letterhead
Clinics care about what the patient takes home. The software prints in two ways: on pre-printed letterhead paper, where it fills in only the patient's details below your header, or on plain paper, where it prints your full header, footer and a watermark itself. The top margin is adjustable, so it lines up with the paper you already have. The doctor's name and registration number go on every slip.

When you should buy something else
